Treatment Teams
Recognizing individual needs, motivational sources, and life circumstances, clinicians at The Lindner Center of HOPE develop flexible strategies to achieve treatment goals with the patient in mind. This is accomplished through the treatment team. Treatment teams form the foundation of patient care at The Lindner Center of HOPE. Treatment teams consist of members from a variety of disciplines including psychiatrists, psychologists, nurses, social workers, occupational/recreational therapists and dieticians. This ensures a comprehensive treatment strategy that will nurture a patient’s biological, psychological, and spiritual needs.
On the inpatient and live-in services units, the treatment teams meet with the patient on a regular basis to develop and implement an individualized treatment plan that focuses on the resolution of symptoms, management of risk factors that contributed to the hospitalization, and addressing discharge needs including assistance with transition of care to their outpatient providers and addressing family, vocational/educational and housing needs as appropriate.
Individuals on the outpatient treatment teams also work collaboratively and seamlessly. They meet weekly to address the ongoing needs of the patients on their team. Each of the patients will be assigned a primary clinician who will be in charge of developing and championing the treatment plan with the patient. Treatment plans will be reviewed on a regular basis by all clinicians involved in their outpatient care. The focus of outpatient treatment plans will be similar to the inpatient plans discussed above.
